The Assisted Living Program provides eligible seniors with personal support services, enabling them to continue living independently (or with an informal caregiver) for as long as possible.

Who Can Participate?

Seniors aged 65+

Who :

    • Live in a private home or residential setting (e.g., apartment) but cannot live safely with current supports.
    • Need daily access to personal support services throughout a 24-hour period.
    • Can direct their own care or have a substitute decision-maker.
    • Are medically stable and pose no risk to themselves or others.

Services Provided

  • Light housekeeping
  • Safety and reassurance checks
  • 24-hour urgent response
  • Short escorted walks
  • Social contact

Priority is given to

  • Hospitalized high-risk seniors
  • Seniors at imminent risk of hospitalization or long-term care admission.

Meals & Dining

Nourishment and connection come together through our dining services.

Social Dining

Enjoy delicious meals, entertainment, and great company at our social dining events. Volunteers serve meals alongside engaging activities for a fun, memorable experience!

Transportation: Free rides via our accessible vehicles are available.

Supper Socials

Join us on the fourth Tuesday of every month at the Almonte Legion for a community meal served by the Ladies Auxiliary.

Cost: $15/person

Entertainment: Live performances at each Supper Social

Extras: 50/50 tickets available!

Cancellation Policy: Kindly provide 48 hours’ notice if you need to cancel. Late cancellations will be charged the full price to cover event costs.

Foot Care Clinics


Caring for your feet is an essential part of overall health. Our foot care specialist provides expert care for nails, corns, calluses, and general foot health during our weekly clinics at the office. If you’re unable to visit us, in-home services are also available.

Vial of Life Program


This lifesaving tool helps first responders access critical medical information during emergencies. The Vial of Life is a specially labeled container that stores details about your pre-existing conditions and treatments.

Cost: Free! Pick up your vial at our office.

Fall Prevention Program


If you’ve experienced a fall, or are at risk of falling, our Falls Prevention Program is here to help. These 12-week programs are offered free of charge from September to June and focus on reducing risks and building confidence.

We’re also continuing to offer virtual fitness classes so you can stay active and connected from the comfort of your own home.

The Station in Carleton Place

The Town of Carleton Place, in collaboration with Carebridge, provides an Active Living Centre for seniors at the former Train Station building (132 Coleman Street, Carleton Place). This community offers in-person programs tailored for seniors.

Get Involved

We’re also continuing to offer virtual fitness classes so you can stay active and connected from the comfort of your own home.

Fit as a Fiddle

Great for beginners, an option for those with limited mobility and range of motion or balance issues. A complete workout sitting in a chair with cardio, strength training, and stretching.

Strength & Balance

Strength & Balance involves a combination of cardio, strength, balance, and flexibility exercises. All you need are running shoes and some light weights.

Black Raven Tai Chi

This class helps people to improve balance, both mentally and physically, which helps significantly reduce the rate of falls experienced by older adults. Other benefits include improving relaxation, vitality, posture, and the immune system.

Upbeat Cardio

Upbeat fitness for seniors, suitable for all levels of mobility (seated modifications provided). From disco to salsa, you’ll love the music, light cardio, strength, flexibility and balance exercises, led by a Personal Trainer and Healthcare Worker for optimal safety.

Gentle Fit

Low impact aerobics, and exercises designed to build muscle strength, improve balance and promote flexibility. Suitable for seniors requiring a less intense and more gradual approach to working out. The ability to walk with or without a mobility aid is necessary but many of the exercises can also be done in a chair. Information about how to avoid falls and what to do if one falls will be provided on a weekly basis as part of the program.

Chair Yoga

Chair yoga incorporates seated and standing poses while focusing on movements designed to improve flexibility, strength, and balance. This practice promotes overall well-being and can be easily modified to suit varying fitness levels

Relaxing Yoga

This is a class aimed at beginners. It is a gentle yoga class in a supportive environment and is accessible to all. There will be lots of options for each pose, allowing each person to move at their own pace.

Respite Care Program

Our respite care program is a fee-for-service offering that provides essential support to seniors who do not qualify for home and community care services.
